Google Forms Share By QR Add-on

Add-on lets you quickly encode URL of the form into a QR code. This QR code can be then used to access the from from mobile devices by scanning the QR code.

Options

Size - size of QR code

Add to Form - if enabled QR code will be added to the form

Save to Document - if enabled QR code will be saved into a Google Document. You can edit the document and print out if needed.

Save as PNG - if enabled QR code will be saved as PNG file on Google Drive

Shorten URL - if enabled long form URL will be shortened using goo.gl shortner resulting in more readable QR code. You can see then statistics of each shortened URL in goo.gl interface.
